Your First Visit

During your first visit, a permanent medical record will be started and you will be asked to fill out several forms, including important emergency telephone numbers. Please try to arrive 15 minutes early to allow extra time to complete the forms.

We try hard to stay on schedule, however, emergencies and patients with unusually complex problems may cause delays. We appreciate your patience and understanding when delays occur and we will be happy to reschedule your appointment if you are unable to wait.

If you are not able to keep your appointment or will be unavoidably delayed, please notify our office as soon as possible.

Your insurance company requires us to collect a copay which we ask that you pay at the time of your office visit. The fee for a cosmetic consultation is $100.
